Registration Junior Competition (10-17 years)

Dear participants,

To take part in the MENTOS SPLASHDIVING WORLDCHAMPIONSHIP 2009 Junior Competition you must be between 10 and 17 years old.

The competition takes place in 2 rounds from the 5m dive board.
The Splashdowns potato, brauni and open L-vis are excluded from the Junior Competition because of safety risks.

Registration is between 28th July and 13th July 2009 (17:00). Registrations after that time cannot be accepted.

At the competition itself the rules of participation and a questionnaire about your health must be signed. You must also show an official identity card. Further more a legal guardian must sign an agreement of participation. To be able to participate the registration fee must be paid at the check-in.

NOTE:

After sending the registration a new window is opened. Within three days you will receive a confirmation from SPLASHDIVING International.

All information that is asked about yourself such as the information you give in the questionnaire about your health will be handled confidential and not given to any third party. We need and will only use this information for the preparation of and for the competition!

Health Declaration:
I accept the personal potential risks of your participation in a SPLASHDIVING competition at your own risk. Freestyle Acrobatic is with the correct behaviour and safety measures a safe sport. If however the existing safety rules are not obeyed, as with every sport increased dangers exist. I accept that various health factors could influence my safety in this sport, for example obesity, asthma, circulatory or respiratory problems, epilepsy, cardiac conditions, acute influenza, abdominal problems. I agree that a medical examination would be advisable to exclude any relevant pre-existing medical conditions. Should one exist I agree to its disclosure.
Further reasons not to take part in the sport would be alcohol and drug consumption (legal or illegal).
